Nurse Manager/MDS
Montevideo MN
The Nurse Manager oversees the care coordination of our residents. They work in conjunction with Nursing Leadership to ensure regulatory compliance as well as ensure the highest quality of care is being provided to residents.


Job Function and Responsibilities:
  • Coordinates and completes the MDS and ensures the RAI process is being followed
  • Review and update resident care plans
  • Assist with supervision and development of nursing activities and personnel
  • Attends and leads care conferences for residents
  • Prepares for and assists with provider rounds
  • Follow-up on labs, orders and ensure accurate documentation
  • Provide family notification and updates regarding residents
  • Attends nursing department meetings
  • Attends IDT, Leadership and QAPI meetings
  • Attends weekly Medicare meetings
  • Assists with QAPI projects and other nursing department projects
  • Assists with policy and procedure development
  • Assume administrative responsibility as delegated by the Director of Nursing
  • Help on the floor when necessary. This may include evenings and weekends.
  • Provide support to nursing with their daily tasks when needed
  • Coordinates and reviews Medicare coverage for residents
  • Stay current with federal, state, and local regulations
  • Stay current with resident rights
  • Rotation schedule for on-call nurse
  • Other duties as assigned
Qualifications:
  • State of Minnesota RN License
  • Experience in long-term care preferred
  • Experience with the MDS or RAI process preferred, but not necessary

Luther Haven provides health care, housing, and other services for older adults in a non-profit, faith-based environment.  Luther Haven is a 70 bed nursing home with a 20 unit market rate assisted living facility.  The non-profit church owned facility is located in Montevideo, MN.  The nursing home and assisted living facility are both connected to the Chippewa County Montevideo hospital.

Job Description
Managing with the Board of Directors, the Nursing Home Administrator strategically repositions, leads, and directs the overall operation of the organization in accordance with resident needs, governmental regulations, and company policies with the main objective of maintaining quality care for the residents and achieving business and financial profitability.

Qualifications:
  • Bachelor’s degree in nursing home administration or related field, or the equivalent combination of education and experience;
  • 3-5 years of management and leadership experience in a skilled nursing care setting;
  • Must possess a valid MN Nursing Home Administrator license;
  • MN Assisted Living Director License is preferred;

Luther Haven has an opening for 1 part-time Care Helper for our day shift.  Responsible for non-direct resident care needs such as passing water, serving coffee, passing trays, transporting residents, making beds, and light housekeeping (such as picking up juice glasses & coffee cups that may be in resident rooms).

Requirements:
  • Ability to work independently after training.                                             
  • Possess a calm, friendly, courteous attitude.
  • Be able to accomplish assigned work in a specific time frame.

Copper Glen, our Assisted Living facility with 20 units, is looking for two Home Care Aides to fill two different part time shifts.  No certification needed...training will be provided. It is working with the elderly doing some daily cares and activities.
