Montevideo, MN: Luther Haven is currently seeking CNAs who are interested in working at our campus. Certified Nursing Assistants will be responsible for providing assistance with activities for daily living (ADL) and restorative care to residents in a skilled nursing home setting. This includes assisting with eating, bathing, grooming, transferring, toileting, checking vitals, answering call lights, and providing care as directed. Requirements:

• Certified Nursing Assistant certification is required.
• Must be able to communicate well with residents, family, and other staff.
• Provide residents with high-quality personal care and services while maintaining consistency with regulations and established best practices.
• Document services are performed on computerized charting systems.
• Working with older adults and those with special needs while maintaining good customer service, patience, and compassion.
• Ability to stand for long periods of time and requires moderate to heavy lifting, bending, and pushing.
• Ability to read and follow directions.
• Ability to complete multiple tasks and prioritize.
• Ability to work well with others and be a team player.
• Must be able to complete and pass a background check (e-mail required).

Shifts: 

• Days (6:00 am - 2:30 pm)- Both full-time and part-time openings
• Evening (2:00 pm – 10:30 pm) – Both full-time and part-time openings
• Nights (10:00 pm – 6:30 am) - Both full-time and part-time openings

Includes every other weekend and a rotation for holidays. 

$2.00/hour evening shift differential. 

$3.00/hour night shift differential.

Montevideo, MN: Luther Haven is a 70-bed skilled nursing facility that has opportunities for both part-time and full-time LPNs interested in making a difference and enriching the lives of our residents at our campus as a Charge Nurse. An LPN helps provide direct nursing care to our residents. Recent graduates are welcome!

Requirements:

• Individuals must have a current/active LPN license with the state of MN.
• Ability to monitor and coordinate resident care.
• Performs medical treatments, procedures, and medication administration with accurate documentation and according to care plan and department policies and procedures.
• Working with older adults and those with special needs while maintaining good customer service, patience, and compassion.
• Demonstrates ability and skill to communicate and maintain effective working relationships with residents, families, co-workers, other healthcare professionals, and members of the community.
• Adheres to federal and state regulations, including Resident Rights.
• Document services performed on computerized charting systems.
• Function independently and as a team.
• Must have good communication and computer skills.
• Ability to multitask and prioritize.
• Detail oriented.
• Must be able to complete and pass a background check (e-mail required).

Night (10:30 pm – 7 am); includes every other weekend and a rotation for holidays 

Hours per pay period: Full-time (64 - 80 hrs/pp) & Part-time (16 - 56 hrs/pp)

Montevideo, MN: The Director of Nursing provides overall direction and leadership for the nursing team at our care center. They are responsible for and work with clinical nursing leaders to coordinate, implement, and evaluate the care and services delivered to our residents.

Job Functions

• Oversees the nursing team that provides care and services to residents.

• Plans, organizes, implements, and evaluates policies, procedures, and services for the nursing department.

• Conducts department and facility-wide quality projects and attends QAPI meetings.

• A member of the leadership team and attends various facility meetings.

• Assists the scheduler with scheduling of the department.

• Assists with hiring of personnel, training, performance, evaluation, and discipline of the nursing department staff.

• Assists clinical leaders with conducting employee reviews and evaluations.

• Provides oversight to clinical leaders regarding the resident assessment process.

• Administrative accountability and responsibility of charting and documentation of medication administration, treatment administration, and cares.

• Coordinates various surveys initiated by the Department of Health (annual survey, case mix audit, quality of life audit and any OHFC survey).

• Initiates investigations regarding Vulnerable Adult issues and appropriate reporting of any such event. 

• Maintain good communication with healthcare professional to ensure the highest clinical outcomes are achieved.

• Understand and adhere to all resident rights and local/state/federal regulations.

• Follow facility policies and ensure department follows them as well.

• Collaborates on and participates with Emergency Preparedness.

• Oversees Infection Control at the facility.

• Responsible for learning, understanding, and implementing legal and regulatory mandates as it affects residents, their care and staff.

• Other duties as assigned.

Qualifications

• Have a valid state RN license

• Leadership experience is preferred

• Must have a thorough knowledge of MDS processes and excellent clinical skills.

• Experience in a long-term care setting preferred

• Must have a solid understanding of federal and state regulations and guidelines affecting nursing home care.

• Needs to be able to establish a good rapport with residents, families and employees

• Must be proficient with computers and programs such as email, scheduling software, and electronic health record.

• Must be able to pass a background check.

Luther Haven has an opening for a part-time evening Dietary Aides.  The Dietary Aide is an essential role in the overall dining services offered to our residents and tenants.  Dietary Aides assist in the preparation, distribution, and supply of meals, snacks, and beverages throughout the day as scheduled, and as requested.
